ADHD interview cheat sheet

One landscape page, split into boxes. A 30-second about-me at the top. Four or five stories as trigger words, with the result at the top of each box. A strip for the questions you want to ask, and their name.

This is a note-taking sheet, not medical or legal advice. The longer explanation, including reasonable adjustments under the Equality Act 2010, is in the guide to filling in the sheet by Nancy Osazee, Founder. It opens in a new tab.

Optional. The downloads on the left stay available either way. We do not send a separate file from this form yet. Saving your email means we may write later, only as the box describes.

How to use the boxes

  1. Write the about-me as a few words, not a speech. You only need about 30 seconds.
  2. Pick a win, a conflict, a mistake, and a time you led. Add a fifth only if you trust it.
  3. Put the result at the top of each box. Three or four trigger words underneath. Never a full sentence.
  4. Add two or three questions and the interviewer's name on the strip.
  5. Colour-code by theme, or print in black and white if you are low on ink.
  6. On a video call, stick the sheet just under the camera. Say each story out loud a couple of times.

Questions

Do I have to give my email to download the sheet?

No. The colour PDF, the black-and-white PDF, and the page you can type into are on this page. The email form is only if you want CareerPilot to keep your address and write to you. The box starts unticked.

Is this medical advice?

No. It is a way to lay out interview notes on one page. It does not diagnose ADHD or tell you whether any law applies to you.

Can I use it on a video call?

Yes. Stick the sheet just under the camera so your eyes stay near the lens. Use trigger words, not a script in a notes app beside the window.

What if I only have three stories?

Use three. Leave the other boxes blank. An empty box is better than a story you do not believe.
