Our client based in Stoke-on-Trent is seeking an experienced Social Worker to join their Hospital Social Work Team. This is an excellent opportunity for a dedicated practitioner to work within a Home First service, supporting adults to transition from temporary care arrangements into sustainable long-term support that promotes independence, wellbeing, and positive outcomes.
The successful candidate will work within a fast-paced multidisciplinary environment, undertaking assessments, care planning, and discharge-related interventions to ensure individuals receive the right support at the right time. Working closely with health professionals, care providers, carers, families, and partner agencies, you will play a key role in supporting safe transitions from hospital and temporary placements into longer-term care arrangements.
This role requires a confident Social Worker who can manage a varied caseload, work flexibly across community and office settings, and contribute to duty responsibilities whilst delivering high-quality, person-centred social work practice.
Key Responsibilities
Core
duties include:
Managing a caseload of adults requiring support through hospital discharge and Home First pathways.
Undertaking Care Act 2014 assessments to identify eligible needs and support long-term planning.
Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence and wellbeing.
Supporting individuals to transition from temporary care arrangements into appropriate long-term support services.
Working collaboratively with health professionals, care providers, carers, families, and partner agencies to coordinate effective discharge and support arrangements.
Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 20

Assessing and managing risk appropriately, ensuring safe and proportionate interventions are implemented.
Identifying opportunities for reablement, rehabilitation, and preventative support to maximise independence and reduce ongoing dependency on services.
Undertaking safeguarding interventions and responding to safeguarding concerns in accordance with Adult Safeguarding legislation and local procedures.
Participating in the team duty rota and responding to urgent referrals and service demands when required.
Completing home visits and community-based assessments to ensure individuals receive appropriate support and care.
Maintaining accurate, timely, and legally compliant assessments, reports, support plans, and case records.
Participating in multidisciplinary meetings, discharge planning discussions, safeguarding meetings, and case reviews.
Qualifications & Experience
Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
Registration: Must be registered with Social Work England.
Experience: Significant post-qualified experience within Adult Social Care services.
Hospital Experience: Experience working within Hospital Social Work, Discharge Teams, Home First Services, or similar Adult Social Care settings is highly desirable.
Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
Assessment Skills: Proven ability to undertake Care Act assessments, support planning, reviews, and risk management interventions.
Safeguarding Experience: Experience undertaking safeguarding enquiries and managing safeguarding concerns.
Multi-Agency Working: Strong experience working collaboratively with NHS professionals, care providers, community services, carers, families, and partner agencies.
Communication Skills: Excellent communication, report-writing, analytical, negotiation, and decision-making skills.
Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
Flexibility: Ability to attend the office a minimum of two days per week, undertake home visits, and participate in the duty rota as required.
