Social Worker - Community Health & Social Care

Liquid Personnel is recruiting Social Worker for its client's Community Health & Social Care Team at Devon. You will manage a varied caseload of adults with complex needs, delivering high-quality, person-centred social work practice. Working closely with health colleagues, carers, families, and partner agencies, you will play a key role in safeguarding individuals and promoting independence within the community. What will your responsibilities be? Undertake strengths-based assessments and develop personalised care and support plans Manage a varied and complex caseload Complete safeguarding enquiries and risk assessments Apply legislation including the Care Act 2014 and Mental Capacity Act 2005 Work collaboratively with health professionals and partner agencies Maintain accurate and timely case recordings Make informed, defensible decisions that deliver positive outcomes Benefits: £35 per hour Full-time contract Supportive and experienced team environment Opportunity to make a real impact within the local community Qualifications & Experience: To be successful in this role you must have, Social Work England registered Social Worker Relevant experience within Adult Social Care Strong assessment, safeguarding, and risk management skills Ability to work independently and as part of a multidisciplinary team Excellent communication and relationship-building skills Why Liquid Personnel?
