Family Practitioner – Children with Disabilities | 11030 | Permanent Contract | 37 hours per week | £35,570 to £37,563 per annum | Grade H | North, East Norfolk & Broadland (based in County Hall, Norwich)   We are recruiting a case holding Family Practitioner to work within our Children with Disabilities, North, East and Broadland Team.  The successful candidate will manage and be accountable for a caseload of vulnerable children, young people and families to provide targeted support and challenge as part of an integrated approach to support families where their child has a significant or complex disability.  About us:  Our Children with Disabilities service provides support to children and young people with substantial and permanent disabilities 0-18 years of age. We have three teams which cover Norfolk and they are multi-disciplinary consisting of social workers, occupational therapists, occupational therapist assistant practitioners and assistant practitioners.
