Your role and responsibilities As a Senior Occupational Therapist, you will: * Support the Home First team with assessments, reviews and interventions. * Work alongside customers to identify strengths, goals and outcomes that promote independence and wellbeing. * Build positive, trusting relationships with customers, their families and carers to deliver personalised support. * Collaborate with social care colleagues, health professionals and voluntary sector partners to provide joined-up, integrated care. * Contribute to a service that focuses on prevention, recovery and enabling people to live safely and independently at home for as long as possible.
