You will undertake holistic assessments, develop tailored intervention plans, and recommend equipment, adaptations, and strategies that support daily living. Working closely with children, families, carers, health professionals, housing providers, and other agencies, you will ensure that children receive the support they need to thrive at home and within their communities. Your work will include advising on major home adaptations, specialist equipment, moving and handling solutions, and housing needs. Managing a varied caseload of moderately complex and complex cases, you will use your professional judgement to assess needs, manage risk, and deliver effective outcomes. You will contribute to reports, reviews, case conferences, and multidisciplinary working, while supporting families to navigate changing needs and future planning.
