Job Description What you'll do As a Hospital to Home Care Assistant, you’ll support people following their discharge from hospital, helping them settle safely back into their own homes and regain their independence. You will: Support people to remain safe and independent at home Provide personal care with dignity and respect Assist with medication, mobility and mealtimes Offer companionship and reassurance following a hospital stay Help people rebuild their confidence and daily routines Work closely with your local team to provide reliable, person-centred care Qualifications What we're looking for No previous care experience? No problem. Full training and support will be provided.
